Health 360 is a one-year program to prevent type 2 diabetes in people that are prediabetic or at risk for developing type 2 diabetes. 1 in 3 American adults have prediabetes which means their blood sugar levels are higher than normal, but not quite high enough to be diagnosed with diabetes. Prediabetes can lead to conditions like heart disease, stroke, and type 2 diabetes. Often, prediabetes can be reversed. Other factors that increase your risk for type 2 diabetes are weight, age, family history, physical activity level, race or previous diagnosis of gestational diabetes.
If you’re prediabetic or at risk for type 2 diabetes, Health 360 can help you turn your risk around. In Health 360 we meet once a week for the first 4 months, then once or twice a month for the remaining 8 months of the program. Each session lasts for one hour.
Health 360 is focused on helping you make moderate changes to your lifestyle through increased physical activity and improved nutrition and eating habits to help you prevent or delay type 2 diabetes. Throughout the program you will have the support of your fellow group members as well as a trained Lifestyle Coach to guide you through the program.
